One grand challenge is to create a transportable coordinated health, mental health, and social service care delivery system. Poor communication between systems results in poorer quality of care, redundancies, lack of mobility for individuals who may need care giving in another state, and lower quality of lives. Improving communication between systems may do much to improve our fragmented service delivery system (Reed, 2013). Use of medical homes has been one means of doing so (Kaye & Townley, 2013).
